To receive information on patient “rights and responsibilities” prior to receiving medical treatment.

To receive a full explanation on treatment options including the risks, side effects, alternatives, costs, and benefits of each option and the ability to make the decision of your choice.
To communicate information in a confidential manner.
To have the right to access information on the type of services, their quality assessment, and quality improvement projects.

To have your personal dignity respected.
To be protected from abuse or harassment.
To enjoy personal privacy and a safe, clean environment, and the ability to restrict visitors or phone calls.
To receive visitors, unless the health care provider states that it would compromise your treatment.
To ensure confidentiality of identifiable health information

To be interviewed and examined in surroundings designed to assure reasonable visual and auditory privacy.
To have a person of one’s own sex present during certain parts of a physical examination, procedure or treatment

To give or withhold consent (say “yes” or “no”) to being recorded, filmed or photographed.

To have a full understanding of how to make a complaint and the complaint resolution process.

To give full and accurate information about health history, medications and status to healthcare team.
To report changes in condition and symptoms including pain.
To ask questions and follow instructions
To ask questions or acknowledge when you don’t understand the treatment plan.
To speak up to any member of the healthcare team when you have any concerns about your care.
To accept actions if care is refused or if the providers’ instructions are not followed.
